Mandatory Medical Certificate Information for Stage 6
The school's official medical certificate is to be used to support an application for extension or illness/misadventure on the basis of illness or injury. This document provides the additional information to best support the application in comparison a generic medical certificate provided by most health professionals.

Year 11 2025 Subject Selection Information
Each student is provided with a summary of courses that are available in 2025-2026. Provided preferences are currently being used to create subject lines for 2025. Students and parents will be advised when they can submit requests for changes to subjects towards the end of Term 4 2024.
HSC 2025 Assessment Handbook
Each student is issued with a copy of the Year 12 HSC Assessment Handbook at the start of Term 4 when their HSC year officially commences. The handbook contains policies, procedures and rules in relation to internal assessment in Stage 6 at The Ponds High School, as well as the assessment schedules for all Year 12 HSC courses. Students are responsible for familiarising themselves with the contents of this handbook. In the event of a change to the handbook or assessment schedule, students will be provided with a copy of the updated version.
The below Assessment Handbook is for the Year 12 cohort completing the HSC exams in 2025.

Assessment Forms
The Ponds High School has procedures in place in which to support students when there is a genuine cause that impacts their ability to meet assessment requirements. The procedures involve the student completing and submitting the relevant form available on the Assessment and Reporting section of the school's website. These assessment forms are for use by students in all grades and courses at The Ponds High School.
Leave Applications
Once a Request for Leave Application has been approved by the Principal, a copy of that application should be attached to this form and taken to class teachers and head teachers to complete. Please note:
- The Request for Leave Application is only available as a hard copy form to be collected from the Office during office hours
- These forms must be completed at least 10 days prior to leave being taken
- Leave should not be taken when there are assessments due, otherwise zero marks may be awarded
